Kansas offers several ways to pay a relative or friend for in-home care. Below are the two most common paths: Medicaid waivers and grant-funded programs. Each includes multiple options with different eligibility requirements.
Medicaid Waivers
These programs cover the full cost of approved in-home care for individuals who meet both functional and financial criteria. Common KS waivers include FE, PD, and BI, depending on the person’s needs.
Grant-Funded Programs
For individuals who don’t qualify for Medicaid but may still be eligible for state or federally supported services. Many of these programs are overseen by the Area Agency on Aging (AAA) and may be based on age, care needs, or income.

They can either be enrolled in—or become eligible for—a Medicaid waiver that allows self-directed care (such as the FE, PD, or BI waiver), or Guardian Home Care of KS can hire their caregiver directly under an agency-directed model with different oversight and rules.

The care seeker must be a Kansas resident
They must be age-eligible, typically 65+ for FE, 16-64 for PD or BI.
The person receiving care must choose in-home care and select their caregiver.

They must complete necessary enrollment and assessment steps through their MCO or case manager.

The caregiver must be an adult (18+) who is physically and mentally able to provide care.
They must not be the care recipient’s spouse or legal guardian (with limited exceptions).
To be paid through Medicaid-funded programs, the chosen caregiver must also meet basic eligibility requirements.
In Kansas, a family member, close friend, or trusted individual can qualify as a paid caregiver if they meet the following criteria:

Your steps depend on how the care will be funded. Below is a walkthrough for getting started through a Medicaid waiver or through grant-funded care.

Apply for KanCare (Kansas Medicaid)
To receive waiver-funded care (including the WORK Program), you must first apply for KanCare, Kansas Medicaid. You can tap or click here to visit KanCare's application page.
Complete a Waiver Eligibility Assessment
Once approved for Medicaid, the state will assign an assessor who'll evaluate your medical and functional needs to determine if you qualify for a waiver. Be sure to name Guardian Home Care of KS as your chosen provider when you speak with them.
Service Authorization From Your MCO
Once you're found eligible, your MCO will authorize services. For WORK participants, authorization and service coordination may go through us as your agency. Let them know that you've chosen Guardian Home Care of KS as your agency, so they can authorize us to begin services.
In-Home Assessment and Intake With Us
Once authorized, we'll schedule a visit to complete paperwork and perform a nurse/admin intake assessment at your home.
Start of Services
After documentation and approvals are finalized, we’ll coordinate directly with your MCO case manager, help with paperwork, and match you with a qualified caregiver—or support the family or friend caregiver you choose.
Consultation Call
Call us or request a callback. We’ll learn about your situation and refer you to the right Area Agency on Aging (AAA) based on your county.
AAA Screening & Assessment
Your local AAA (Area Agency on Aging) will conduct a needs assessment and determine if you qualify for programs like: Older Americans Act (OAA) Senior Care Act (SCA) They’ll look at factors like age, functional limitations, and income.
Approval & Care Plan Setup
If you qualify, the AAA will approve services and may help establish a care plan. They will also coordinate with Guardian Home Care of KS as the selected provider.
Start of Services
Once we receive authorization and guidance from the AAA, our team will complete intake paperwork, assign a caregiver, and begin services according to your care plan
